CE certificate will be emailed within 10 days to social workers who sign in and out of each session attended and complete session evaluations—including an overall conference evaluation. Joint Accreditation Statement 
In support of improving patient care, this activity has been planned and implemented by the Postgraduate Institute for Medicine (PIM) and ACMA. Postgraduate Institute for Medicine is jointly accredited by the American Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team. 

Physician Continuing Medical Education (CME) 
The Postgraduate Institute for Medicine designates this live activity for a maximum of 18.5 AMA PRA Category 1 Credit(s)™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

 In order to obtain CME credits, attendee will need to complete an online evaluation. This will be completed through CME University. An email will be sent to all Physicians immediately following the conference. 

Continuing Nursing Education (CE) 
This educational activity for 23.5 contact hours is provided by the Postgraduate Institute for Medicine. 

Nursing and Social Work CEs: To obtain CE Certificate, attendee will need to complete an online evaluation of attended sessions. Evaluation notification will be sent to via email to the address used for registration within 7 days of the conference. 

PLEASE NOTE:
 Attendees are responsible for verifying that the approving organizations listed above are accepted by their state’s practicing board. Unless otherwise noted on the CE certificate, attendees are also responsible for submitting their earned continuing education credits/certificates to the appropriate accrediting and/or certifying organization(s) to maintain their individual certification(s), recertification(s), licensure(s), etc. As required by applicable boards, a copy of this certificate and evaluation form will be retained by ACMA for a minimum of 7 years after the course concludes.
